You always want to be aware of that, you'll hook large fish in spots where you don't expect them.
On every drift you should be expecting a big fish to take your fly and plan accordingly. How will you set the hook (in every part of the drift), what angle will you initially apply pressure (if you can quickly get downstream pressure on a big fish it is more likely to head upstream), which way will you head downstream if necessary? If you are prepared for a big fish you are less likely to get busted off in a frantic attempt to get it in.
This was one of the earliest and most important lessons taken from a Borger presentation (Gary) almost 20 years ago.
